New patient forms

Nobody wants to show up somewhere new and spend the first ten minutes of a visit filling out a clipboard. We'd rather you walk in and feel settled right away, so if you can take a few minutes to complete your forms ahead of time, we'll have everything we need before you arrive and we can just get started. Our team reviews your information in advance so your first visit actually feels like a first visit, not an intake process.
Insurance
We work with most major insurance plans
Insurance can be confusing and sometimes stressful. That is why our team is here to guide you, answer your questions, and make sure you understand your benefits so you do not have to figure it out alone. We accept all PPO insurance plans, and even if we are out-of-network, we will submit claims for you to make the process easy. Do not see your plan listed below? No problem. Financing options
Flexible payment options so cost doesn't stand in the way of your care
Cost is one of the most common reasons people put off dental care they actually need, and we don't want that to be the reason for you. We offer a few different ways to make treatment work for your budget.
- CareCredit third-party financing that lets you spread the cost of treatment over time with flexible payment plans.
- In-house payment plans we offer our own payment plans up to six months, handled directly through our office.

After any procedure, we want you to feel confident and informed about your recovery. Every patient receives a paper copy of post-operative instructions at the office, and now you can also access them online anytime.
